1. To enter, complete and return this Official Entry Form. All entries must be received by November 16, 2007. Mechanically reproduced entries will not be accepted. Grand Prize: a Solvay Glass Sunroom (10’ x 12’, est. retail value $21,000) First Prize Winners Choice of either (a) 8 windows for the home (double-hung or sliding windows) or ; (b) a bay or bow window (est. retail value of either choice $5,000). Second Prize (1): a Solvay Glass tub or shower makeover (est. retail value of $4,000). All prizes include installation. This sweepstakes consists of several separate thematic presentations. Winners will be selected in a random drawing from among all entries received from all presentations. Drawing will be conducted by Solvay Glass. Odds of winning are dependent upon the number of entries received. No purchase or in-home product demonstration required to win. Winners will be notified by phone. Limit one prize to an individual or household. Prizes are nontransferable and no substitutions are allowed. Sponsor assumes no responsibility or liability for damages, losses or injury resulting from acceptance or use of any prize. Taxes, if any, are the responsibility of the individual winners. Winners must take delivery of prize within 1 year of being notified by the distributor. In the event the winner has purchased a product that is all or part of their prize between 2/1/2007 and 11//2007, such prize winner may elect to receive the retail value of that prize in lieu of the product or products. Winners may be required to verify address or execute an affidavit of eligibility and release within 14 days of notification attempt or prize will be forfeited and an alternate winner selected. Entry constitutes (1) permission to use winners' names and likenesses for promotional purposes without further compensation except where prohibited by law, and (2) permission to be called for advertising purposes.
